Welcome

If you recently applied for Avera Health Plans individual insurance through an agent or AveraHealthPlans.com, your application is being reviewed for accuracy. You'll receive a welcome letter and payment coupon in the mail within 10 days from when your application is received.

First Premium

If you didn't pay for your first month's premium during the application process, please return the payment coupon and your payment right away to ensure timely coverage.

Your First Member ID Card

Your member identification (ID) card contains important information about your health plan. Be sure to carry it with you at all times, because you'll be required to show it whenever you need health care services.

Employer Plan Members

If you receive Avera Health Plans insurance through your employer, your member ID card(s) will be mailed to you within 10 to 14 business days after processing the application and setting up your plan benefits.

Individual & Family Plan Members

If you've purchased your own individual insurance, your member identification cards will be mailed to you within 10 to 14 business days after your first payment has been received and cleared.

After you receive your member ID card

After you receive your member ID card, log in to the online member portal or through our Avera Health Plans mobile app for access to all of your personalized plan information. Your member ID card has the information you need to log in and create your new username and password.

Haven't received your member ID card?

If your policy is effective, but you haven't received your member ID card yet, you can contact us to request the information you need to log in. Once logged in, you can view and print an image of your member ID card.

Lost your member ID card?

If you lose your member ID card, you can print a temporary card and order a replacement card by logging in to the member portal and choosing the "CLICK HERE to print a copy or order a replacement ID card" link in green. If you need further assistance, contact us.

Medicare Supplement Members

Welcome Kit

After your Medicare Supplement application has been approved, you’ll receive a welcome kit in the mail that includes your:

  • Medicare Supplement policy
  • Network hospital directory (Select plans only)
  • 24/7 member resources flyer
  • Welcome letter
  • Member ID card

If you lose your member ID card, you can print a temporary one and order a replacement by logging into the member portal. Click the green link to print a copy or order a replacement ID card. For further assistance, please contact us.

Check Duplicate Coverage

It's important to note that when you become an Avera Health Plans Medicare Supplement Insurance policyholder, you cannot have another Medicare Supplement or Medicare Advantage policy.

To avoid duplication of coverage and unnecessary expenses, our Medicare Supplement Insurance policy should replace any other Medicare Supplement Insurance you may have.

Your Medicare Supplement Insurance Member ID Card

After you receive your member ID card

Once you receive your member ID card, log in to the online member portal or through our Avera Health Plans mobile app for access to all of the details specific to your plan. Your member ID card has the information you need to log in and create your new username and password.

Member Portal

How to enroll in the portal

Once your Avera Health Plans coverage is effective, be sure to register for access to the member portal.

  • You'll need your member ID number and group number (both found on your member ID card).
  • If your policy is effective, but you haven't received your member ID card yet, contact us for help.

Give someone access to your portal

  • Each member over the age of 18 can register for his or her own login.
  • If you would like to authorize someone to access your health information via the member portal, you must complete and submit this Authorization for Access of Health Information form (pdf).
  • All members listed on the form must first register for their own account, including the member authorizing the release of information.
